The Making of a Physician Innovator

The project, the environment, and the artifact are only the vehicle. The change in the learner is the most important product — and AI makes that distinction urgent.

Innovation is creativity in action, validated through human experience. Competence is not the polish of an artifact; it is what the learner can now perceive, judge, decide, and do that they could not reliably do before.

01

The progression

A cycle, not a checklist. Each pass returns with greater judgment.
01SEE
Observation
Recognize meaningful opportunities in human experience — before interpreting or solving.
02UNDERSTAND
Human understanding
Grasp the experience and needs of the people affected; separate wants from needs.
03FRAME
Problem definition
Turn observation into a meaningful, evidence-supported problem worth solving.
04CREATE
Possibility
Generate grounded alternatives; resist fixating on the first plausible solution.
05TEST
Experimentation
Put an imperfect idea into contact with reality to reduce uncertainty.
06LEARN
Interpretation
Read what human experience reveals; seek the evidence that disconfirms.
07ADAPT
Iteration
Pivot, persist, or stop on evidence — not attachment to the idea.
08IMPLEMENT
Value creation
Move a validated solution responsibly toward real-world use.
Through repeated cycles, the project becomes the vehicle through which the learner changes.

What the austere environment magnifies

The +2σ of the model — the rare few that constraint forces into the open.
Constraint-activated · μ+2σ When not everything can be done, the consequential few become visible. Constraint is a magnifier — it surfaces what matters most.
DISCERN
Of everything I can observe, what matters?
PRIORITIZE
Of everything that matters, what warrants action?
RESOURCE
Given what actually exists here, what can we responsibly do?
I noticed  →  I discerned significance  →  I prioritized  →  I acted with what was available  →  I observed the consequence  →  I adapted.
03

The proposition

The project provides the vehicle.
The austere environment magnifies what matters.
Human experience provides the validation.
AI provides augmentation.
The change in the learner is the most important product.
Competence must remain with the learner.
04

Put the artifact away

Don't make the curriculum AI-proof — make the evidence AI-resistant.

A sophisticated AI-enabled output proves something sophisticated was produced — not that the learner developed. After substantial AI-supported work, set the artifacts aside and ask the learner to reconstruct the journey. Five questions test whether the learning belongs to them:

  1. 1What did you personally experience?
  2. 2What did you personally notice or understand because of that experience?
  3. 3What did AI contribute?
  4. 4What judgment or decision remained yours?
  5. 5How are you different now because you went through this? The most important question
What an owned answer sounds like

I thought this. Then I saw this. I talked to this person — that made me question my assumption. I considered these alternatives. I chose this because of these constraints. We tried it. This happened. That surprised me, so I changed this. I am still uncertain about that. Therefore, this is what I would do next.

Unpolished by nature — the false starts and changed minds are the evidence. If the reasoning disappears once the artifact is gone, the competence may be borrowed.
